SWVS Enrollment Checklist
2006-2007

Please note:  Before proceeding with the enrollment process, you will need to make sure your home district is a member of the consortium.  You can check our member list using the button on the left, call your local district office, or contact us at SWVS.

       Read through and understand all Southwind Virtual School requirements and the
information contained in the Handbook (follow the links to the left)
       By September 1st, submit a completed enrollment form either online or by mailing the form
to:

       Receive a phone call from SWVS staff to discuss the following:
                   Student and parent compatibility with online schooling
                   Enrollment requirements and confirmation of submitted information
                   Courses and grade levels
                   Student/Parent orientation
                   Textbooks and Computer requirements
       Attend student/parent orientation. 
Bring with you the following:
                   Birth certificate copy (students must be over 5 years old before September 1, 2007)
                   Immunization record
                   Completed physical form (if required)
           
                   $20 for computer lease fee
            During orientation, students and parents will be asked to sign:
                   Acceptable Use Policy
                   Student Compact
                   Parent Compact
       Record transfer request form (if necessary)
       Students will not be given access, materials, or equipment prior to attending orientation.  Once orientation has been completed, students will be able to start their school year on August 17th.
       Meet attendance and engagement requirements prior to, on, and following September 20th.  Failure to meet these requirements will result in being dropped from the SWVS program.
